Trust partnered with CCHI (California Coverage and Health Initiatives) and partner community-based organizations to increase awareness, engage community members to support Medi-Cal enrollment and referrals to social service and other county programs.
The community partners were able to reach deep into communities, share information and resources pertinent to enrollment but also guide and support other social determinant gaps. They were able to translate outreach in 81 languages, gather responses and feedback in native languages, provide a secure and compliant way for community members to engage with community partners.

Trust’s community-first outreach approach, aligning Managed Care Plans, Enhanced Care Plans (ECM) and Community Supports (CS) providers has proven highly effective in engaging hard to reach members, increased ECM enrollment activity and improved closed loop referrals.
Trust’s CalAIM partners, are ECM (Enhanced Care Management) and Community Service (CS) providers, engaging their populations of focus to increase assessments and closed-loop referrals intended to improve the quality of life and health outcomes of this Medi-Cal population. Community Supports programs offered by our partners include housing navigation, deposits, housing sustaining services, recuperative care, medically tailored meals, short term post hospitalization, day habilitation, and asthma remediation.
